Returns, Refunds, and Eligibility

How Eligibility Works

When you place your order with Momentum Health 360, we place a temporary authorization hold on your payment method. Following your submission, you will be prompted to complete a HIPAA-secure medical intake questionnaire.

Our licensed clinical team will then review your profile to ensure the program is a safe and appropriate fit for your health goals.

  • If approved: Your payment will be finalized and processed.
  • If not eligible: The authorization hold will be released immediately, and you will not be charged.

Refund & Return Policy

Because our treatments involve personalized, prescription-grade products tailored specifically to your medical needs, we must adhere to strict safety and pharmacy regulations:

  • Final Sale: Once a licensed practitioner approves your intake and transmits the prescription to the pharmacy, all sales are final. We cannot accept returns of prescription medications once they have been dispensed.
  • No Refunds for Use: We do not offer prorated refunds for partially used subscription cycles or medications already received.
  • Cancellation Fee: If you choose to cancel your order after a clinician has approved your intake form, but before the pharmacy has begun processing the medication, a $50 administrative cancellation fee will be deducted from your refund.

Prescription Cancellation

Once a prescription has been finalized and sent to our pharmacy partners, it enters a non-refundable status. We encourage you to reach out to our support team promptly if you have any questions before your clinical review is completed.
